taxi number (if you have it)
pick up address
destination
whether the journey was a booking, a hail off the street or taxi rank
a description of the article lost. For electronic
devices, such as a mobile phone, laptop computer or video camera, please
include the make model and serial number of the item
Your contact details, including name and telephone
number
